ASHP Recommendations

The ASHP discusses various examples of risk points and diversion methods for controlled waste to help facilities determine the best security measures for ensuring the integrity and safety of controlled substances. Whether you’re working with sharps bins on location or transporting sharps waste offsite, follow these Controlled Substance (CS) storage and security guidelines:
  • Always store controlled substances in locked, secured containers, like lockable cabinets, automated dispensers, or biohazard sharps containers. Only authorized individuals should be able to access these locked cabinets or bins to ensure the safety of healthcare workers.

  • Implement chain-of-custody rules when transporting portable lock boxes or sharps bins to follow the same security measures as onsite containers. Store transportable boxes in locked and secured areas that are only accessible by authorized personnel. These areas should not be used for other purposes.
  • Lock-out times for electronic locks on carts (e.g., medication or anesthesia carts) containing CS are limited ”to the narrowest window of time that is appropriate for the particular setting.
  • There is a defined process to ensure that only authorized individuals have access to CS. Access to CS storage areas is minimized and limited to authorized personnel.
  • There is a complete assessment of all HCWs with access privileges to ensure that only those permitted to access have access (i.e., currently employed, temporary employees, or licensed independent practitioners with privileges), and removal of access privileges occurs immediately upon separation.
